Health dept constitutes committee for enquiry into maternal death at CHC Hajin

Srinagar: Health department has constituted a committee to inquire into the maternal death at CHC Hajan on Friday.

“In view of the maternal death that occurred today, 14-02-2025, at CHC Hajan, it is imperative to conduct a thorough inquiry to assess the circumstances leading to the unfortunate incident and to identify any lapses in patient management.” Reads the order of BMO Hajan,a copy of which lies with news agency JKNS.

“Accordingly, a Maternal Death Review (MDR) Committee is hereby constituted with member including Block Medical Officer Hajan – Chairperson, Dr. Shaheen Chalkoo Consultant Anaesthesia CHC Sumbal – Member, Dr Nisar Ahmad Medical Officer, CHC Hajan] – Member, Nasreena Kounsar, In-Charge Labour Room] – Member and Mohammad Yaqoob Ganaie BM&EO Hajan] – Member

The terms of Reference for the Inquiry will be detailed case analysis of the deceased patient, including medical history, antenatal care received, and any complications noted during pregnancy, Examination of the treatment protocol followed during admission, delivery, and post-operative care, Assessment of the role of duty doctors, nursing staff, and support staff in managing the case, Identification of any medical negligence, procedural lapses, or delays in providing emergency care, Review of hospital records, staff duty rosters, and patient monitoring sheets to evaluate adherence to standard protocols and Recommendations for corrective measures to prevent future incidents and improve maternal care services at CHC Hajan.

The committee shall submit its detailed report within 48 hours to the Block Medical Officer, CHC Hajan, reads the order.

If any negligence or misconduct is found, appropriate disciplinary action shall be recommended as per norms, order reads, adding that findings shall be forwarded to the Chief Medical Officer, Bandipora, for further action.

“This committee is expected to conduct a fair, impartial, and detailed review to ensure accountability and improve maternal healthcare services,” it reads further.—(JKNS)
